How do I fill out the Drug and/or Violent Criminal Activity Consent form?

The purpose of this form is to provide consent to our agency to run any verifications with law enforcement agencies relating to criminal activity. This is a required form. All adult members in the household must sign this form and date the form. Please note the first line is to be filled out by the head of household. The remaining lines are filled out by other adult household members. If you have a live-in aide in your unit they must sign this form in the allotted line at the bottom.
