How do I fill out the Fair Information Practice Act form?

The purpose of this form is to collect information from applicants and participants in regards to their housing to determine eligibility, amount of rent and voucher size. This form is only signed and dated by the Head of Household. Both the date and signature are a requirement as you can see in the example below:
