How do I fill out the Family Responsibilities Form?

This three-page document explains in detail the obligations of the family while participating in the Section 8 voucher program. It is highly recommended to read each obligation in order to fully understand the rules and what not to do under the program. After you have read all three pages, the head of household must sign and date the form. No other member signs this form.
