How to fill out a mutual termination agreement

If you are interested in relocating to another unit and have a mobile voucher, the first step of the process is breaking the lease. You can do this using a mutual termination agreement. To see how to fill this form out, see below. 

The following must be filled out:

• Full printed name of the owner.
• Full printed name of the tenant. 
• The full address the tenant lives in currently. 
• The effective date the lease will be terminated. 
• The signature/date of the owner. 
• The signature/date of the tenant.

If any of the above is left blank, the notice will not be processed and you will not be approved to move.
