The purpose of this form is for our agency to collect information from our applicants and participants that may be used for verification purposes and be provided with permission to obtain and disclose information as part of the Fair Information Practices Act.
This form is only signed and dated by the Head of Household. Both the date and signature are a requirement as you can see in the example below:
