How do I fill out my recertification packet?

As you are required to fill out your recertification annually, it’s important to fill out each document correctly on the first attempt to remove any additional requests or delays. If you only need to correct one document or are only interested in certain documents from the packet, please select the articles below. All documents are required to be filled out correctly in addition to submitting all relevant income/asset information. Select which document you are filling out and follow the directions.

In the event you are a zero income household or individual, you may also need to fill out the additional forms:
