How do I fill out the Statement of Family Obligations form?

This  one page document explains in detail the obligations of the family while participating in the Massachusetts Rental Voucher program. It is highly recommended to read each obligation in order to fully understand the rules and what not to do under the program. After you have read this statement, the head of household must include your name at the top of the form, sign and date the form. No other member signs this form.
